男遊秘魯後遭馬蠅蟲卵寄生嘴唇 手術活捉3隻幼蟲 Yahoo奇摩

33歲陳姓男子去年前往秘魯旅遊，返國兩周後嘴唇莫名腫大且刺痛，外觀近似皰疹，但治療半個月仍未好轉，進一步檢查揪出竟是罕見馬蠅蟲卵寄生，經手術順利活捉3隻幼蟲。

秘魯觀光旅遊近年受世界各地遊客青睞，33歲陳姓男子去年10月走訪知名景點馬丘比丘，並入住當地雨林區民宿，旅遊全程沒有什麼異樣，11月初回國後，陳男發現嘴唇表面有一點小破皮。

他不以為意，沒想到兩周後仍未好轉，嘴唇逐漸腫大且伴隨日益加劇的疼痛感，經皮膚科診所診斷為皰疹後給予口服抗病毒藥物、抗生素及藥膏治療卻不見改善。

陳男轉診到馬偕醫院皮膚科，進行細菌培養未驗出任何菌種，最後轉診至口腔顎面外科，檢查發現竟然是少見的馬蠅蟲卵寄生，且蟲卵寄生在體內已孵化成幼蟲。

台灣過去僅有2例馬蠅蟲卵寄生在頭皮的案例。馬偕醫院口腔醫學部主任劉崇基今天在衛教記者會中分享，這是他行醫以來，首次見到寄生在嘴唇的情況，嘴唇上方及嘴角位置有3個明顯孔洞，還會聽到奇怪聲音，且隱隱約約有物體活動的跡象。

經進一步檢查，「真的是蟲在裡面。」劉崇基說，而且是在嘴唇這麼敏感的部位同時有3隻，曾在門診嘗試夾出，因病人無法忍受疼痛，最後選擇替病人安排於在手術室內全身麻醉，歷經15分鐘手術時間，順利「活捉蟲蟲」，每隻長約2.5公分。

劉崇基提醒，旅遊過程中應隨時留意蚊蟲叮咬，除穿著淺色長袖衣褲，進入叢林區最好戴帽子以保護頸部及頭皮，不擦香水或使用體香膏，裸露的皮膚可選用適當防蚊液並適時補擦，若從高風險地區回國後，身上有不明類似蚊蟲叮咬傷口，又遲遲未癒合，應盡早就醫，並主動告知旅遊史。
