以下範例所描述的研究工具為問卷調查，包括受訪者的背景資料、選擇題和開放性問題等，共分為五大部份: 

1. The questionnaire consisted of five sections, the first of which was intended to elicit demographic information on the respondents. 2. The second section asked respondents to rate their degree of familiarity with statistical terms. 3. Section 3 focused on attitudes regarding the role of statistically based research. 4. The fourth section is designed to assess opinions of specific problem areas for which there is no consensus on appropriate procedures. 5. More open-ended research problems were presented in the fifth section, where respondents were asked how they would go about solving these problems and dealing with these data. 6. Finally, survey respondents were also asked to make general comments on the questionnaire. 

此問卷由五大部份所組分，第一部份得知受訪者背景資料。第二部份請受訪者就統計術語的熟悉程度評分。第三部份探究對於統計研究所扮演角色的態度。第四部份評估受訪者對適當程序缺乏共識所產生的問題有何意見。第五部份則是開放性問題，請受訪者回答他們如何解決問題並處理這些資料。最後，請調查的受訪者對問卷提出評論。 

l 講解:

1. The questionnaire consisted of five sections, the first of which was intended to elicit demographic information on the respondents.

    第1句先講問卷由幾個部份所組成，consist of是最常用的動詞片語。而逗號後的半句是非限定關係子句，以the first of which作為主詞。which指的是前面的先行詞five sections，因此the first of which就是the first of the five sections，亦即問卷的第一部份。因為問卷的第一部份通常都是調查受訪者的背景資料，因此全句除了修改問卷組成部份的數量之外，基本上是可以直接引用到自己的論文寫作上。 

2. The second section asked respondents to rate their degree of familiarity with statistical terms.

第2句之後逐句說明問卷各組成部份的內容或目的。此句要求受訪者作程度上的評分，注意它是用主動語態asked。 

3. Section 3 focused on attitudes regarding the role of statistically based research.

第3句也是用主動語態focused on，另外regarding也是常用的介系詞，引介出此部份問卷的內容或主題，可以代換成concerning。 

4. The fourth section is designed to assess opinions of specific problem areas for which there is no consensus on appropriate procedures.

第4句說明問卷的第四部份，注意fourth的拼法，很多人用電腦寫作常打成*forth而不自覺。而這句是用被動語態is designed。 

5. More open-ended research problems were presented in the fifth section, where respondents were asked how they would go about solving these problems and dealing with these data.

這句也是用被動語態were presented和were asked。逗號後的半句是由關係副詞where所引導的形容詞子句來修飾其前的先行詞the fifth section，說明問卷第五部份問題的內容。 

6. Finally, survey respondents were also asked to make general comments on the questionnaire.

最後一句用連接副詞finally開頭，繼續說明第五部份的問題，也是用被動語態were asked。

問卷調查工具的寫法可由以下句型組成，需要對組成問卷的各個部份逐一說明。:
