If you or your spouse has earned 40 credits through paying payroll taxes at work, you will not need to pay premiums for Medicare Part A benefits (mainly coverage for hospital stays). But without these credits, you still have the option to buy Part A by paying monthly premiums. You don’t need any credits to qualify for Part B (doctors’ services, outpatient care, medical equipment) or Part D (prescription drug coverage) if you’re a U.S. citizen or a legal resident who’s lived in the United States for at least five years. You just pay the required premiums for these benefits, like anybody else.

It depends on your circumstances. If you’re covered beyond age 65 by health insurance provided by an employer for whom you or your spouse actively works (and the employer has 20 or more employees), you can delay Medicare enrollment until the job ends, without risking late penalties. Otherwise (even if you have retiree benefits or COBRA), you need to sign up for Medicare no later than three months after your 65th birthday to avoid permanent late penalties and delayed coverage. (Note that other enrollment rules may apply if you have insurance from a small employer, you live abroad or in prison, or you are not a U.S. citizen.)

If you’re already receiving Social Security benefits when you turn 65, you don’t need to do anything. Social Security will automatically enroll you in Medicare Parts A and B and send your Medicare card through the mail three months before your 65th birthday. But if you don’t yet collect these retirement benefits, you must apply for Medicare by contacting Social Security, which handles Medicare enrollment. You should apply either during your seven-month initial enrollment period, of which the fourth month is the one in which you turn 65, or—if you’ve delayed signing up because of having insurance from current employment—during a special enrollment period of up to eight months after the job ends.

What are the consequences if you don’t enroll in Medicare Part B at the right time for you?

If you miss your enrollment deadline, you can sign up only during a general enrollment period, which runs from January 1 to March 31 each year, with coverage beginning July 1 of the same year. You’ll get late penalties, permanently added to your monthly Part B premiums, if you’ve gone for more than 12 months without health insurance from current employment (your own or your spouse’s) since turning 65.

Medicare covers many medical and prescription drug costs, but not all. It never covers the day-to-day expenses of living in a long-term care facility such as a nursing home or assisted living. Original Medicare does not cover routine dental, hearing or vision care, but some Medicare Advantage plans (HMOs and PPOs) cover these services, sometimes as a separate benefit package for an extra premium.

You can review and change your Medicare coverage during Medicare’s open enrollment period, which runs from October 15 to December 7 each year. During this time, you can switch from Original Medicare to a Medicare Advantage HMO or PPO, or vice versa; from one Medicare Advantage plan to another; from one Part D drug plan to another. The new coverage begins January 1. You can also change coverage at other times of the year in specific circumstances—for example, if you move out of your current plan’s service area; move into or out of a nursing home; receive low-cost drug coverage under the Extra Help program; or sign up for a plan that has received Medicare’s top five-star quality rating. Also, people joining Medicare for the first time get their own enrollment periods.
